House Calls Program
If you or a loved one finds it difficult to leave home due to a medical condition or mobility challenges, the Loyola House Calls Program may be the right fit for you.
Loyola offers in-home primary care for individuals living in parts of west suburban Cook County who are homebound. This includes people living with conditions such as dementia, chronic illness, or limited mobility that make traveling to a doctor's office difficult or unsafe.
Who We Serve
Our program is designed for individuals who:
- are homebound due to medical, cognitive, or physical limitations
- live in designated areas of west suburban Cook County
- are enrolled in certain Medicaid or Medicare plus Medicaid plans
What We Offer
Our dedicated care team includes:
- a primary care physician
- nurses
- a social worker
- a pharmacist
Together, we provide:
- management of chronic health conditions such as heart failure, diabetes, COPD, high blood pressure, and dementia
- medication refills and immunization
- helping obtaining medical equipment
- advance care planning to support future health decisions
More than Medical Care
As a House Calls patient, you'll also gain access to a network of community partners who can help address non-medical needs that impact health, including:
- food access
- housing support
- behavioral health services
- senior services and more
